Dorset County Council are seeking to appoint an innovative, forward thinking, and well-organised individual to undertake the role of Area Practice Manager (Transitions).

You must be passionate about making a real difference to the lives and futures of young people with likely needs as adults and their families. If you are not already employed within a leadership role then you will need to be able to clearly demonstrate emerging leadership skills.

In Dorset, we are committed to working in partnership - through a dedicated project - with young people, their families, education settings, providers and employers to identify and develop the right services and approaches which will enable young people transitioning to adulthood to reach their potential and live fulfilling lives. We have dedicated resources to enable us to achieve these objectives, and commitment from leaders in Dorset County Council and partner organisations to getting our Transitions approach right. We are passionate about working in a person-centred way, with real creativity, to enable young people with care and support needs to live healthy and independent lives, to engage with their communities, and to have rewarding occupations.

We know that providing leadership in this context can be challenging, but we are optimistic and positive about the difference that we can make to the lives of young people moving into adulthood and their families. You must have a degree in Social Work or equivalent, be registered with the HCPC and be able to fulfil the travel requirements of the post.
