Job Description
A permanent vacancy has arisen for a Social Worker or Occupational Therapist within Stirling Council's Social Work Hospital Discharge and Intermediate Care Team, 35 hrs per week.
We are looking for a Qualified OT/SW (registered with the HCPC/SSSC) for 35 hrs per week to support the team to undertake Outcome Focussed Assessments and to support adults to be discharged from hospital in a planned and coordinated way either to their own home or to a care setting of choice, with appropriate services in place to minimise risk and maximise independence.
The hospital team work to a two week assessment period, work in a pressured environment with tight timescales and with regular scrutiny from partner agencies to avoid delayed discharges.
Although Hospital Discharge work will take priority there will also be a requirement to undertake assessments within the Short Term Assessment units across Stirling to work in collaboration with health colleagues to coordinate and assess longer term needs. Knowledge and competence in assessing and managing complex discharges and risk is essential. The successful applicant will assist in maintaining vulnerable adults in the community by providing a range of social care and care management tasks. This includes carrying out Outcome Focussed Assessments, supporting adults in the use of Self Direct Support and reviewing and monitoring support packages and carers assessments. We are looking for an autonomous and dynamic practitioner to advocate for peoples' outcomes despite the pressured timescales and challenges from other professionals and families. Knowledge and experience in AWI and ASP procedures is also essential as responsibilities will also include statutory work under the Adults with Incapacity Act and Adult Support and Protection Act.
