To ensure we are best placed to take full advantage of the opportunities this brings, we have restructured our Adult Social Care service to help deliver our aim of transforming the lives of the people of the borough, by investing in services to support them in achieving their aspirations. As a result of our new focus and approach, we have created the following position within the service:

You will work across the full range of Adult Social Care services, working with health colleagues and other partners to provide support for GPs and ensure that our communities have access to the services they need and want.
